All referral's are handled by our Triage Coordinator who will forward to the most appropriate scheme. We provide support with:
- A wide range of information and advice on issues that affect older people, their carers and families
- Benefit checks and form filling
- Practical support around the home – to include shopping, cleaning and ironing
- Support with managing Personal Budgets
- A wide range of information, advice and support for people with dementia, their carers and families
